Judson Terrace Homes and Judson Lodge partner with Wilshire Health and Community Services

July 20, 2021 -- Judson Terrace Homes and Judson Lodge, HumanGood affordable housing communities in San Luis Obispo, CA, have partnered with Wilshire Health and Community Services to deliver in-residence medical care for our residents. Wilshire Health and Community Services is a mission-driven organization with deep roots and a “big heart” in San Luis Obispo.

The new partnership provides in-home health services in residents’ apartments through Wilshire Connected Care, a primary care medical practice, that enables residents to more easily access health care. Resident service coordinators at both communities coordinate care with Wilshire Health. Program costs are covered through Medicare/Medi-Cal and the residents’ medical insurance, and no personal medical information is shared to protect residents’ privacy. Wilshire also provides home health and hospice services, when appropriate.

“By providing primary medical care in the home we are able to see patients where they live, avoid unnecessary trips to the doctor or emergency room, and stabilize their medical and living situations through our constant case management,” said Dr. Kevin Parzych, Wilshire’s chief medical officer. “Our wrap around services such as transportation, peer counseling and friendly visits also provides opportunities for seniors to be less isolated and have some assistance around the home. All of this contributes to a better overall quality of life in keeping with the mission of both of our organizations.”
