What to Bring to Plymouth Village

During your short-term, post-acute rehabilitation stay at Plymouth Village, you’ll be up, dressed and active—so pack clothing that is comfortable and easy to get on and off.

Also, feel free to bring a computer for use with our free Wi-Fi, books you’re reading and one or two outfits you might want to wear for socializing or lounging in.

What is post-acute care?

Short-term rehabilitation—or post-acute, transitional care—helps patients regain strength and relieve pain while recovering from a health event, such as a hospital stay for an illness, injury or surgery. The goal of short-stay rehabilitation is to help you get back to your home and routine quickly. 

How will I get from the hospital to Plymouth Village?

Transportation will be arranged for you. Plymouth Village will be informed of your anticipated arrival time, and your family and or emergency contact will be notified by your hospital case manager/discharge planner.

What will happen once I arrive at Plymouth Village?

Our team will be waiting for you to arrive, and we will get you settled in your room as we get to know you while completing the admission process. Please ask any questions you might have along the way.

When do I start my post-acute rehabilitation therapy?

Our therapy services are available seven days a week, and you’ll typically begin rehabilitation from day one with an evaluation. After your evaluation, we’ll create a customized care plan focused on your individual rehabilitation goals.

How long will I be at Plymouth Village's short-term rehabilitation center?

Your length of stay at Plymouth Village will vary, depending on your own personal rehabilitation goals. You will receive a tentative discharge date when you arrive, and we’ll adjust that date as needed throughout your rehabilitation stay.

What do I do if my doctor can’t come to Plymouth Vilalge?

In some cases, your physician might not be able to check in on you during your short-term post-acute rehabilitation stay. In these instances, we have expert physicians who will be able to work with you. After your discharge from Plymouth Village, you will need to follow up with your primary care doctor.

Where will I eat my meals?

We have multiple dining options at Plymouth Village. You are welcome to eat socially in our dining room, enjoy our bistro, dine with friends and family or have a meal delivered to your room.

What activities can I participate in during my short-term rehabilitation stay?

In addition to your regular therapy program, you are also welcome to the endless opportunities and activities available to the Plymouth Village community.

What if I need extra help or care when I get home?

Our social worker can answer any questions you have and help you access community resources and services.
